Cordarone 150mg/3ml Injection 6's is a prescription antiarrhythmic medicine containing Amiodarone Hydrochloride, used in hospital or specialist settings for the treatment of serious cardiac rhythm disturbances when other therapies are ineffective, unsuitable or cannot be used.
Each 3ml ampoule contains 150mg of Amiodarone Hydrochloride, equivalent to a concentration of 50mg/ml.
Cordarone may be used for severe tachyarrhythmias including supraventricular, nodal and ventricular tachycardias, atrial flutter, atrial fibrillation and rhythm disturbances associated with Wolff-Parkinson-White syndrome. It may also be used for ventricular fibrillation and in cardiopulmonary resuscitation for cardiac arrest associated with ventricular fibrillation that remains resistant to defibrillation.
Intravenous Amiodarone is particularly useful when a rapid antiarrhythmic response is required or when oral administration is not possible. Because Amiodarone can significantly affect heart rate, cardiac conduction and blood pressure, treatment must be initiated and monitored in a hospital or specialist setting with appropriate ECG and cardiovascular monitoring.
Cordarone may be administered by intravenous infusion or, in selected emergency circumstances, by slow intravenous injection according to the treating physician's protocol.

How to Use:
• Cordarone Injection must be administered only under hospital or specialist supervision.
• Continuous ECG and blood-pressure monitoring should be available during treatment.
• Intravenous infusion is generally preferred to direct IV injection because rapid injection can cause significant haemodynamic effects.
• A commonly recommended IV infusion loading dose is 5mg/kg body weight diluted in 250ml of 5% dextrose solution.
• The loading infusion is generally administered over approximately 20 minutes to 2 hours according to the patient's clinical condition.
• Additional infusions may be prescribed according to therapeutic response and specialist protocol.
• In extreme clinical emergencies, 150–300mg may be given as a slow IV injection after appropriate dilution with 5% dextrose.
• A slow IV injection should be administered over at least 3 minutes.
• A direct IV dose should not generally be repeated for at least 15 minutes.
• For cardiopulmonary resuscitation in ventricular fibrillation or pulseless ventricular tachycardia resistant to defibrillation, a 300mg IV dose may be used according to resuscitation protocols, with an additional 150mg dose considered if the arrhythmia persists.
• Cordarone IV should be diluted with 5% dextrose solution.
• Do not dilute with normal saline because Cordarone is incompatible with saline.
• Repeated or continuous IV infusion is preferably administered through a central venous catheter to reduce local injection-site reactions.
• Follow the treating cardiologist's prescribed dose and monitoring protocol exactly.
Precautions:
• Prescription-only antiarrhythmic medicine.
• Treatment must be initiated and monitored by an experienced physician.
• Do not use in patients with known hypersensitivity to Amiodarone, iodine or any component of the formulation.
• Contraindicated in certain patients with severe sinus bradycardia, sinoatrial block, sick sinus syndrome or significant atrioventricular conduction disorders unless appropriate pacing support is available.
• Avoid use in patients with known thyroid dysfunction unless specifically assessed and managed by a specialist.
• Amiodarone may prolong the QT interval and can interact with medicines that increase the risk of torsades de pointes.
• Correct significant electrolyte disturbances, particularly low potassium, before or during treatment as clinically appropriate.
• Amiodarone may cause severe bradycardia, conduction disturbances or worsening arrhythmias.
• Rapid IV administration may cause severe hypotension or circulatory collapse.
• Monitor ECG, heart rate and blood pressure during intravenous treatment.
• Thyroid function, liver function and other relevant parameters may require monitoring according to treatment duration and clinical circumstances.
• Use cautiously in patients with hypotension, severe heart failure or cardiomyopathy.
• Inform the anaesthetist if the patient is receiving Amiodarone before surgery.
• Cordarone IV contains benzyl alcohol and requires special caution in infants and young children.
• Pregnancy use is generally avoided except in exceptional circumstances where the specialist determines that the benefit outweighs the risk.
• Breastfeeding is generally contraindicated during Amiodarone therapy.
• Store below 25°C.
• Keep ampoules in the original container.
• Protect from inappropriate heat and storage conditions.
